Summary: Responsible for providing direct nursing care as ordered by physician, outlined in the plan of care, and in accordance with standards of nursing practices and regulations. Takes charge in leading and direct ing resident/patient care on assigned unit. Directly supervises Certified Nursing Assistants to ensure assignments and care are complete based on professional standards.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties as assigned.
ï‚· Personally participates in the evaluation and delivery of care including medication administration, treatments, Adl care, etc. as ordered by physician and outlined in the plan of care (according to scope of practice)
ï‚· Conduct regular rounds to monitor resident activity and ensure resident/ patients receive quality care based on professional standards and facility policies
ï‚· Evaluate residents physical and psychosocial status
ï‚· Communicate with residents, family members, physicians and supervisor regarding resident status.
ï‚· Maintain up to date and accurate medical records during tour of duty on assigned unit.ï‚· Coordinate and prepare for new admissions, transfers, discharge of residents
ï‚· Transcribe physician orders
ï‚· Assist in updating the plan of care that addresses the needs of the residents/ patients
ï‚· Complete incident/ accident reportsï‚· Understands, complies with and promotes all rules regarding resident rightsï‚· Maintain safe and clean work area
ï‚· Ensures proper storage of equipment and supplies
ï‚· Ensure Infection prevention and control practices are followed on assigned unit
ï‚· Report hazardous conditions, incidents and accidents, and allegations to supervisorï‚· Attend orientation and inâ€servicesï‚· Assist in orientation and training
ï‚· Attend staff meetings
ï‚· Perform other duties as assigned or designated.
SUPERVISORY RESPONSIBILITIES : Lead, monitor, and assume charge of personnel in the delivery of assigned resident care on assigned unit. Directly supervises Certified Nursing Assistants to ensure assignments and care are complete based on professional standard s. Accomplishes retention results by communicating assignment,planning, monitoring, appraising positive results, counseling, and disciplining staff under direct supervision.
QUALIFICATIONS : To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION and/or EXPERIENCE ï‚· Completion of Licensed Practical Nursing programï‚· Long Term Care/ Skilled Nursing Care experience preferred
CERTIFICATIONS, LICENSES, REGISTRATIONS ï‚· Must be a Licensed Practical Nurse or Registered Nurse in good standing and currently licensed by the stateï‚· CPRâ€BLSï‚· Valid State issued Identification
KNOWLEDGE, SKIL LS AND ABILITIES ï‚· Excellent written and verbal communication skillsï‚· Ability to multiâ€task while being detail orientedï‚· Intermediate computer skills
