CRAssociates, Inc. currently has an exciting projected opportunity for a Clinical Nurse Disease Manager to provide services at Columbus Air Force Base in Columbus, MS!
The Disease Manager RN will utilize major components of population health and disease management, ensuring healthcare interventions for individuals and populations of patients with prevalent and/or chronic diseases in order to achieve disease management compliance, improved overall quality of life, and reduced healthcare costs. Also expected to perform clinical nursing care in the execution of disease management services.
Qualifications:
- Must have an Associate Degree in Nursing or Bachelor of Science in Nursing by a nationally accredited nursing program.
- Active, valid, current, and unrestricted state license to practice as an RN in any US state/jurisdiction.
- A minimum of 3 years (Associate Degree in Nursing) or 2 years (BSN) of full-time experience in management of patient populations with prevalent and chronic diseases.
- Prefer individuals that are already certified, but at a minimum must be eligible for certification in a Disease Management related area or a patient education body. Examples:
- Chronic Care Professional (CCP) by Health Sciences Institute.
- Certified Health Coach (CHC) by National Society of Health Coaches.
- Certified Diabetes Educator (CDE) by National Certification Board for Diabetes Educators.
- Certified Asthma Educator (AE-C) by National Asthma Educator Certification Board.
