The Medical Case Manager coordinates resources and creates flexible, cost-effective options for catastrophically or chronically ill or injured individuals on a case-by-case basis to facilitate quality individualized treatm! ent goals, including timely return to work if appropriate. Areas for nurses: Jackson, Meridian, Hattiesburg, Vicksburg
Essential Job Duties: Responsible for assessment & coordination of medical care for injured workers. Works closely with adjusters, medical providers, employers & attorneys to ensure appropriate care to medically rehabilitate individuals & return them to work. Makes recommendations regarding health care resources. Provides assessment, planning, implementation, and evaluation of patient's progress. Evaluates patient's treatment plan for appropriateness, medical necessity, and cost effectiveness. Attends physician appointments, hospital and/or long-term facility discharge planning conferences, for the purpose of determining appropriateness of care and developing an effective long-term care strategy. Regional travel required on field cases. Reliable transportation required. Requires regular and consistent attendance. Extensive documentation usin! g various computer programs. Communication is frequent and com! pleted by using telephone, email and computer programs.
